Health Form Requirements
The following Health Forms are required by Camp Highroad (see details below). Both forms should be mailed or brought to camp no later than check-in on the first day of your child’s/children’s week of camp. Your child must have valid, current Health Forms in order to attend camp.
Form #1 relates to a child's past health history and pages 1-3 must be filled out and signed by a parent or guardian and given to camp staff at check-in.
Form #2 is based on information from a new health exam done specifically for the camp. Form #2 does NOT have to be turned into camp if a similar health form, based on a physical exam (sports, school, etc.) that has taken place in the 24 months prior to the child's stay at camp, is provided instead. If you do not have records from a previous examination, please use Form #2 and schedule an examination for your child. A licensed medical provider's signature is required regardless of which form is provided, our Form #2 or a similar one, as mentioned above.

We realize that filling out these forms can be time-consuming, however your children's safety at camp is of the utmost importance to us. We absolutely need to know your children’s health history and current health status so that we can make educated decisions during their time at camp.
